We are one of 17 Beacon Communities established in the United States by The Office of the National Coordinator (ONC) for Health Information Technology. We are part of a larger health care revolution that demonstrates how health IT investments can advance patient-centered care.

Our goal is to build and strengthen community level health IT infrastructure and exchange capabilities through the use and testing of innovative new approaches to performance measurement, technology integration, and the delivery and coordination of care.

We are working to deliver measurable improvements in health care cost, quality and population health.

Our efforts are focused on the use and adoption of health IT that results in improved diabetes care and self-management in the cities of Detroit, Highland Park, Hamtramck, Dearborn, and Dearborn Heights.

Collectively, our work embodies a scalable foundation that can be duplicated and used for the care and management of other disease types in the future.
